Job Description Provide direct medical care, monitor clients’ health status, and support their recovery journey Conduct physical and mental health assessments Monitor overall health, identify risks, and implement appropriate interventions Communicate assessment findings to the interdisciplinary team Administer medication and monitor for side effects or adverse reactions Wound care and emergency medical management Support detox processes, monitor vital signs and symptoms Educate clients on health, nutrition, exercise, hygiene, and wellness Facilitate group/individual health education as part of the program Collaborate with case managers and other healthcare professionals Maintain accurate medical records Implement infection prevention and control measures Guide on handling medical waste and health hazards Provide client support and advocacy
